It was never specified what the "accident" was, only that the Icarus
exploded, killing everyone onboard.

Possible (fan-wank) answer:  The last time, Sheridan stayed onboard to
run the battle from the command center, Ivanova went out in a Starfury
to lead the fighters and Garibaldi stayed onboard to lead the Security
forces against boarders.

This time, Sheridan isn't there so Ivanova stays onboard to run the
battle and Garibaldi takes a Starfury to lead the fighters and Zack
leads the Security forces.
