I think you are right on both counts.  ISTR hearing something about that
at some point.  Quinine water is something else that isn't easily found
any more, too.

Ideally, and I expect and demand it now, the doctor-patient relationship
should be one of equal participation... the patient brings the knowledge
of the body in question, and the doctor the medical expertise... and the
two are essentially equal in importance.  Any doctor that doesn't expect
me to participate in my care, and listen to my input, doesn't remain my
doctor for long.

More of the "insurance-run" medical "care"...  A certain amount of
standardization of precedures to be followed isn't a bad thing, gives
guidelines for how to procede and all, but one-size-fits-all is just so
patently wrong...  More likely the incidence of malpractice is lower
because some of us just aren't litigious and would rather just find a
better doctor... or people are getting fed up with lawyers as much as
they are with doctors, or more...  What's left probably does think that
"evidence-based" conformity makes the difference... maybe once things
hit the courts it does?
