I don't know how Duke compares in size to Unity.  Unity's about the
smallest of the hospitals right here in the Rochester area.  There are a
couple of regional hospitals further out in the area that are a little
smaller, but the other local ones (Highland, RGH, and Strong) are all
somewhat larger, especially Strong which is part of the UR Medical
Center.  A smaller hospital allows the food to get to the patient floors
faster and more efficiently... at least potentially...  

The only meal I saw that could have stood a lot of improvement was the
hamburger, dry puck on a white roll, served with a slice of tomato and a
slice of lettuce at the side to put on top.  Even that was reasonably
edible, though, according to Richard... I didn't try that one...   

It did appear that the food had been freshly cooked, and done pretty
much from scratch.  One supper was lasagne rollups, a lasagne noodle
rolled around a cheese filling like for manicotti, with a tomato sauce
that tasted like it was made from fresh tomatoes, or at least very good
canned ones... it had a nice fresh taste to it, and more body than your
typical marinara sauce...  The night before, he'd had a goulash that he
was guessing had to be fresh made, that tasted pretty good, too.  And
these were the low sodium dishes...  :)

Even the other local hospitals have been producing fairly good meals,
at least from what I've seen in the last few years at Highland and
Strong... with others who've been admitted... I think there's someone
local that's been lobbying for better food, that happens to be in a
position to do something about it...  :)
