Most things are probably still in flux... can't be really settled until
things are further along... especially since you said his rehab is
likely to take a while, due to all the issues...

Things are still probably somewhat trial and error there... with things
so out of control when he went in, I'd expect it not to be all that easy
to figure out the final reasonable dosage... so low blood sugar episodes
are probably to be expected.... Getting real ice cream out of it does
sound like low blood sugar, possibly from over-dosage of insulin...
