My sister Tait, who goes by her middle name, has had similar
experience... She had gone by E. Tait for most legal stuff, but has had
to use the first name for some few government things..  The hospital
insisted on using my dad's first name at one point... since he was a
junior, he'd never used his first name, only his middle name... usually
only with the first initial where needed...

Yes, I do think you've mentioned that before...  ;)  Was the credit
union more careful after that...?

I ran into some one with the same name as mine using the same
pharmacy... her address showed up on one of my meds, which is how I
discovered it...  ;)  Fortunately, it was caught before either of us got
the wrong meds... or the records became conflated...  ;)  But I've
noticed that lately, one has to give both the address and the day/month
of birth when picking up meds now... probably a good practice...  :)
