We can only have one elected president at a time.
But if you follow the numbers, you will notice we
have had 44 presidents (including Obama) but have
really had only 43 presidents (including Obama).
You see, Americans are flunkies in math, and can
barely count to ten.  That is why one president
was counted twice (he was elected to two terms,
but not two consecutive terms).  And not all of
our presidents were elected.  Gerald Ford comes
to mind.  And George W. Bush.

The U.S. Constitution makes no mention of numbering presidents.
None of the amendments to the U.S. Constitution makes mention of
numbering presidents.  Numbering presidents is simply a way of
listing presidents.

Lyndon Johnson only served one term in office, chosing not to
run for a second term.  Prior to serving his first and only
term, he had to take the oath of office as acting president due
to the death of President John F. Kennedy.  In 1967, Johnson
issued an executive order changing the term from "acting
president" to "president", so that historians would not call
him "acting president" following JFK's death.  An amendment
to the U.S. Constitution made Johnson's executive order moot,
which is why Gerald Ford became "president" rather than "acting
president" upon Nixon's resignation from office.

If a president becomes incapacitated and the vice president
has to assume the duties of the presidency, the president is
technically no longer president.  Which is why Ronald Reagan
made sure he was completely conscious when doctors operated
on him after John Hinckley's assassination attempt.  And
GWB remained conscious when doctor's examined his brain (er,
colon).

There are also other examples of a president being incapacitated
for short periods of time, with the vice president having to assume
the duties of the presidency.  Not everything is spelled out in the
U.S. Constitution and its Amendments.  Nor is it possible to spell
out everything.
