Run your own mail server.  Thus they would have to intercept all port 25 
and 110 traffic.  Use TLS and encrypt it with SSL.

Encrypt the payload.  Again they would have to monitor all port 21 and 
port 22 traffic on their network to do this.  It can be done but it is a 
big task.

Yes, most ISPs attempt to proxy HTTP either directly or via WCCP.  E-
mail and basic HTTP can be done fairly easily with current technology.  
SSL and many other protocols are much harder to do because the ISP 
doesn't have a proxy or server to use as a monitor point.

The average ISP doesn't have the resources or money to implement a 
solution to monitor all traffic.  The technoids on the Internet can find 
ways around the monitoring as fast as the ISPs deploy it.  Simple port  
level redirection will work to stop port level monitoring of common well 
know ports.  Again, as you have pointed out, ISPs can deploy some pint 
solutions for some protocols but a full blown capturing solution that 
captures all session and payload data on all ports across an ISPs entire 
network is not feasible with today's technology.
