Changing tack to fit your example, I heard a gaffe so bad on XM Radio the 
other day that I was moved to e-mail the news director (who happened to also 
be the on-air commentator who made the gaffe).  He subsequently thanked me 
for the information.  He was doing a short, in-between-news-and-commercials 
pre-recorded bit, which was a roundup of the famous people who'd died this 
year.  He'd say the name, and a short clip of one of their performances would 
follow.  This went on and on for some time, until I heard him somberly intone 
"Maureen Stapleton," followed by...Jean Stapleton (who is very much alive) 
shrieking out "...songs that made the Hit Par-aaaade" in Edith Bunker's 
voice.

Astonishing that no one on his research staff managed to catch that.
