Nobody else has used them because they're afraid of the consequences.

Our doctrine has been that we will not use nukes first, since the end of WWII.

All these little pissant countries that want nukes think somehow that is going
to enable them to accomplish their goals.  All they have though, is those
nukes.  They don't have anything to back them up.  If North Korea uses a nuke,
then what do they do?  They don't have enough troops to occupy another country
and make it stick.  Neither anybody else, except us.  And we have vowed never
to use nukes unless we are attacked with them first.

Nukes are weapons of revenge and annihilation.  We realized that in 1945, which
is why we have the policy we do today.  The only country that is justified in
having nukes and in possibly using them is Israel.  They are surrounded by
enemies who have sworn to exterminate them.  If they are backed into a corner
and faced with being utterly destroyed, then they are justified in using
whatever means at their disposal to prevent that.
